Unlike synthetic folic acid, which requires multiple enzymatic conversion steps to reach this active form, L-5-MTHF is immediately bioavailable regardless of MTHFR genotype. This distinction is clinically critical: individuals with common MTHFR C677T or A1298C polymorphisms have reduced capacity to convert dietary or supplemental folic acid to L-5-MTHF, and may derive little benefit — or even experience adverse effects — from standard folic acid supplementation. The formula uses levomefolate calcium (Merck's pharmaceutical-grade calcium salt of L-5-MTHF), which also avoids masking vitamin B12 deficiency — a recognised limitation of high-dose folic acid supplementation.
Methylcobalamin (Mecobalamin) — 1,000 mcg Methylcobalamin is the active, methylated form of vitamin B12 that functions directly as a methyl donor in the methionine synthase reaction — the same reaction that depends on L-5-MTHF. Unlike cyanocobalamin (the synthetic form used in most lower-cost supplements), methylcobalamin requires no hepatic conversion and is the form naturally found in human plasma and cerebrospinal fluid. At 1,000 mcg per capsule, this provides a therapeutic dose appropriate for supporting remethylation capacity in patients with confirmed or suspected B12 depletion, impaired B12 absorption, or elevated homocysteine.
Choline (as Choline Dihydrogen Citrate) — 300 mg Choline supports methylation through the betaine-homocysteine methyltransferase (BHMT) pathway — an alternative homocysteine remethylation route that operates independently of folate and B12, and is particularly active in the liver. In this pathway, choline is first converted to betaine, which then donates a methyl group to convert homocysteine to methionine. The BHMT pathway provides meaningful methylation reserve capacity when the folate/B12-dependent methionine synthase pathway is under demand or impaired — making choline an important complementary methyl donor rather than a peripheral addition to the formula.
Activated B Vitamin Cofactors — R5P and P5P
Riboflavin-5'-Phosphate Sodium (R5P) — equivalent to Riboflavin (B2) 25 mg Riboflavin-5'-phosphate is the activated coenzyme form of vitamin B2 (FAD/FMN precursor). Riboflavin is an essential cofactor for methylenetetrahydrofolate reductase (MTHFR) — the enzyme responsible for converting 5,10-methyleneTHF to 5-methylTHF (L-5-MTHF). Inadequate riboflavin status directly impairs MTHFR activity and reduces endogenous folate activation, compounding the methylation deficit in MTHFR variant carriers. Research has demonstrated that riboflavin supplementation can meaningfully improve homocysteine levels, specifically in individuals with the MTHFR 677TT genotype. The activated R5P form bypasses hepatic conversion from riboflavin, making it the appropriate form for patients with impaired conversion capacity.
Pyridoxal-5'-Phosphate (P5P) — equivalent to Pyridoxine (B6) 20 mg P5P is the active coenzyme form of vitamin B6, required for the transsulfuration pathway — the metabolic route through which homocysteine is converted to cysteine (and ultimately glutathione) rather than remethylated to methionine. This pathway is the primary route for homocysteine clearance when the methionine cycle is under load, and represents the detoxification arm of homocysteine metabolism. Adequate P5P is therefore essential for complete homocysteine metabolism across both the remethylation and transsulphuration arms of the cycle. P5P also serves as a cofactor for neurotransmitter synthesis (serotonin, dopamine, GABA) and numerous other enzymatic reactions.
